Transforming Care
Homes not Hospitals
Our service is working towards the transforming care programme and has contributed with several successful hospital discharges. We believe individuals with learning disabilities, autism and/or mental health conditions need to be living in the community, with the right support and close to home.
Our community service has been developed to provide enhanced care for individuals with complex presentations. Our therapeutic pathway ensures individuals receive clinical interventions with 24-hour care and support to work towards greater independence as equal citizens of the community.
Bespoke packages of care
- Person centred, strengths-based and outcome-oriented approach
- Experienced team of professionals able to facilitate TCP discharges for adults and children
- Immediate therapeutic input following principles of positive behaviour support
- In house multi-disciplinary team offering clinical support, intervention, and training
We have extensive experience and training in assisting secure inpatients with their return to the community, such individuals may be subject to Part Three of the Mental Health Act and/or others including:
- S.37/41 – Hospital Order with restrictions
- S.37 – Guardianship Order
- S.41 – Restrictive Patient
- S.45A&B – Hospital and Limitation Direction
- Community Treatment Order (CTO)
- S.37 – Hospital Order
Our goal is to help these individuals set the stage for a happy and fulfilling life. This is accomplished through hiring and training competent support workers to provide meaningful support and encouragement, respect and dignity, choice and control, and work towards achieving each individuals’ personal goals.
