We provide high-quality specialist care and personalised support packages for individuals with Learning Disabilities, Mental Health conditions and/or Autism. Our services are tailored to meet each individuals’ needs and aspirations, to promote greater independence and a more fulfilling life.
AIMS AND OBJECTIVES
Community-Based Support
The aim of our Community-based support service is to prevent relapse, lower and safely manage behaviour amongst people with a learning disability and/or autism that may put themselves or members of the public at risk, which would otherwise lead to contact with the criminal justice system or admission to a secure hospital.

Our Goals and Aims
we offer the following support and services for our service users:
- Outreach support to facilitate safe, well-managed and timely transition into the community from secure settings
- Consultation and liaison with multi agencies, statutory services and/or other services
- Forensic risk assessment and formulation, risk management and liaison
- Support in sourcing accommodation under long term assured tenancies
- Support in employment, education and access to meaningful activities
- Delivery of offence specific treatments/interventions, addressing of physical violence, sexual violence, fire-setting etc.
- Support appropriate admissions to secure hospital and continue to be actively involved with the patient journey with a focus on rehabilitation and recovery and a return to the community
WHO WE SUPPORT
Person-Centred Support
At Assisted Living Care, we support individuals with Learning Disabilities, Mental Health conditions and/or Autism who have come in contact with the criminal justice system or may be at risk of such contact. These individuals often ‘slip between the cracks’ in existing services; they are frequently excluded from mainstream mental health or forensic services due to their learning disability and/or autism and from learning disability services due to being deemed too able or ‘too high risk.’
We provide a wide range of high-quality person-centred care and support services that have evolved to meet the needs of individuals with autism, their families, caregivers, and autism professionals.
We assist individuals with learning disabilities who have a variety of requirements. Because there is such a wide range of learning disabilities, the help we provide differs from person to person.
A large percentage of autistic people and people with learning disabilities experience poor mental health. Our mental health service is designed to allow the users to lead a normal life in the community and to experience fulfilment as an individual.
